Student Forms: English Education - Evaluation for Admission to College of Education

English Education majors use this form to request a confidential evaluation (which is part of their application to the College of Education and Human Sciences) from an English professor. This eForm replaces the paper English Education evaluation forms, also known as the blue sheets. English Education majors must have four evaluations on file--from their professors for ENGL 210; ENGL 221 or 284; another 200-level English course (other than those listed above); and a 300-level English course --when they submit their application for admission to the College of Education and Human Sciences. English Education majors are advised to send this form to their professor before the end of the semester they take the relevant course.

Searching for a list of all eForms?

  1. Navigate to https://eform.uwec.edu.
  2. Log in using your university username and password.
  3. Click the Start New Form tab.
  4. Select the form owner's department from the drop-down menu.

Resuming a started and saved eForm?

  1. Navigate to https://eform.uwec.edu.
  2. Log in using your university username and password.
  3. Click the My Tasks tab to view any forms that require your input.
    NOTE: Must have clicked Save and Close for Later.
  4. Click the View Forms in Progress tab to view a form's progress that you initiated.
