Objective:
The objective of this procedure is to identify the appropriate application and procedures pertaining to Americans with Disabilities (ADA) accommodation requests.

Scope:
The scope of this procedure is to define the steps to follow for requesting an accommodation under the ADA Guidelines.
Statement:
A request for accommodation can be made through the human resources department. The official forms are available through BP Logix and must include the Employee Information and the Physicians Information. All documents must be completed before submitting to Human Resources for processing.
Procedure:

Step | Action
1. | Access the request form by opening BP Logix, click on Start New Form, Select Human Resources, and selecting ADA Reasonable Accommodation Request form.
2. | Completed the Employee Information section including a: Current Job Description, Description of Disability, Description of how disability impairs ability to perform assigned job duties, Description of accommodation being requested
3. | Follow the instructions to receive the Physicians Information document.

3. | Follow the instructions to receive the Physicians Information document.
- When you have completed the upper portion of this eForm, click "Submit" below. The form will remain - you may "Save and Close for Later".
- A Physician's Information PDF will be created.
- It will immediately come to you via email as a PDF. (It will include all information that you entered above.Print the PDF and take to the doctor to be completed.
4. | Submit the documentation. When the doctor has completed the form, scan or take a picture of the form with your camera. The scanned image must be uploaded to the eForm. Find the form at https://eform.uwec.edu / My Tasks Add Scan or Picture of Physician Report
5. | Click "Submit" to send to HR for review and accommodation determination.
